According to an article in Open Minds, in December 2016, New York Medicaid expanded its health home program. With this change came the inclusion of children living with two or more chronic medical conditions, complex trauma or serious emotional disturbances (SED).
In New York, the children’s health home model uses lead health homes. These health homes enter contracts with care management agencies (CMAs), which provide most health home services.
Since 1985, J&D Ultracare has been providing skilled nursing services to the medically fragile population of the New York Metropolitan area as well as the greater Hudson Valley. We see clients in Rockland, Dutchess, Putnam County, Westchester, Sullivan, Ulster, and Orange Counties.
