When you invite someone into your home, you want to feel trust and confidence. Below are some questions that can help guide your decision-making when seeking home care services:
What is the difference between intermittent skilled nursing services and private duty nursing care?
Intermittent skilled nursing services typically involve visits by a nurse to your home as ordered by your physician to aid in your recovery.
Private Duty Nursing (PDN) is defined as the provision of medically necessary, complex skilled nursing care in your home on a fee-for service basis by a Registered Nurse (RN) or a Licensed Practical Nurse (LPN). The purpose of PDN is to assess, monitor and provide skilled nursing care in the home on an hourly basis; to assist in the transition of care from a more acute setting to home; and to teach competent caregivers the assumption of this care when the condition of the individual is stabilized.
Who can order Home Care Services?
Any physician who supports the need for home care services, and who is willing to advocate for the authorization of this service by providing a LETTER OF MEDICAL NECESSITY.
If my family member is currently hospitalized, who can assist me in getting home care?
A physician, hospital discharge planner, case manager, social worker and/or nurse.
If not in the hospital, can I get home care services?
You can contact the Agency directly to make a referral. Your physician MUST order the services.
How do I know if Home Care Services is covered by my insurance?
Contact your insurance company and ask whether your home care benefit allows for skilled nursing visits and / or private duty nursing.
What are the hours of operation?
Regular business hours are Monday – Friday, 8:30am to 5:00pm.
However, over 200 highly trained nurses provide care 24 hours/day, 7 days/week.
Additionally, a scheduler and clinical administrator are available on call around the clock.
If my insurance will not cover home care services but I wish to pay for them, will Ultracare accept private pay?
Yes, consideration is given on a case-by-case basis and will depend upon the type of service requested and availability of qualified staff.
Is a parent / caregiver expected to participate in the care of my family member?
Yes, Ultracare will partner with you to teach and assist you with your family member’s care activities and monitor the progress.
How will I know when it is time to be discharged from Ultracare’s home care services?
Once a patient’s goals have been met and a safe discharge plan has been determined, the physician will provide the discharge order to the Agency.
The nurse will notify you of the physician’s discharge order, and again review / answer questions prior to the time and date of the discharge.
Agency staff will also assist you in identifying other resources that may be appropriate.
