3 1/2 yr.old male, Central Valley NY. Seizure disorder, VP Shunt, Developmentally Delayed & Visually Impaired. Ambulatory & PO Feeds. TRANSPORT ONLY

2 1/2 yr. old male, Blauvelt NY. Spina Bifeda & H/O Hydrocephalus, urinary straight catheterization and bowel regimen. Ambulatory & PO Feeds.

(Evenings)

3 yr. old female, Pine Bush NY. Down Syndrome w/ h/o trachemalacia, hypothyroidism, NPO, Ambulatory, neubilizer, oral suctioning. TRANSPORT ONLY

9 mos. old male, White Plains NY. H/O hypotonia, reflux, anal atresia, & laryngomalecia. Skilled needs for colostomy care, GB enteral feedings, respiratory assess/oral suction, & infant growth development.

( 8 hour days)

2yr. old Central Valley NY. Gaucher Disease, Type I. Trach, Vent dependent, O2, suction and GB feedings. Developmentally delayed.

(Days and Evening shifts)

3yr. old Hyde Park NY. Seizure disorder, H/O apnea,cognitively delayed. GB feeds, BIPAP use while asleep, O2, suction as needed.

(Weekend days 7a-7p/ Weekdays evenings 5p-11p)

2 yr.old Montgomery NY. Seizure disorder & failure to thrive w/ G Button feedings. Non-ambulatory.

12 yr. old Cornwall on Hudson NY. Congenital disorder, H/O seizures w G/Button feedings. Non- ambulatory.

Nights open (10p-5a)

4 yr. old , New Windsor NY. CP/Developmentally delayed & Gbutton Feedings. Non- ambulatory

Weekend days

19 yr. old, Warwick NY. Congenital muscle disorder. CPAP( overnights), & G Button feeds. Non- ambulatory
